A riser has to seal to sound concrete. Where the tank top is spalling or a lid has cracked, that gets addressed first.

Fit the riser to the tank
Sealed to the tank top with the correct adhesive and gasket. A riser that leaks groundwater in is worse than no riser at all.

Set height to grade
Just proud of finished ground so surface water runs away from it rather than pooling on the lid. Not sunk, not sticking up to catch a mower.

Secure the lid
